Why do I have cramps but no period?

Cramps can begin before period bleeding, but belly pain without a period can have other causes too. The feeling alone cannot tell you what is happening or when your period will start. Decide what help you need from the pain and how you feel, rather than waiting for blood to appear.

Sudden or severe belly pain, pain that is getting worse, or feeling faint needs urgent medical help now. Get a trusted adult; do not wait for your period. If you collapse or are too weak to stand, call 911.

You can ask for help with pain before you know what is causing it.

First, describe what is happening now

“Cramps” is a useful description, not a diagnosis. Notice where it hurts, when it began and whether it comes and goes. You do not need to work out which organ hurts before asking someone to help.

A quick note for yourself: Pain started ___. It is in ___. It is coming and going / staying there. I can / cannot do ___. Bleeding has / has not started.

Keep the note on paper or somewhere private if you want to save it. There is no need to fill in every blank or keep a long record before getting care.

Does it mean my period is about to start?

It might: period pain can start just before bleeding. But cramps do not work as a countdown. Bowel or bladder problems can also cause pain low in your belly, and a list of possible causes cannot tell you which one applies.

If you are waiting for your first period, keep a pad handy if that helps you feel prepared. Our first-period signs guideexplains why body changes cannot give you an exact start date. If you have already started having periods, a changing early schedule is covered in the irregular-period guide.

If the pain is mild and you otherwise feel okay

Pause and rest somewhere comfortable. You can sip water. If this feels like familiar mild period discomfort, a warm shower or a covered warm pack used according to its instructions may help. Stop anything that feels worse; comfort measures are not a test of what is causing the pain.

At school, you can ask for help without saying you are definitely on your period. You do not have to wait until the end of class if you feel unwell.

A request without guessing the cause

My lower belly has been hurting since ___. I haven’t started bleeding, and I don’t know why it hurts. Can I go to the nurse or call someone who can help?

When should I get help for cramps without bleeding?

Get medical help now if the pain is low on the right side or stays constant for more than two hours. Tell a trusted adult and ask for urgent assessment; do not wait for your period to start. Sudden or severe pain, worsening pain or feeling faint also needs urgent help.

Contact a clinician within 24 hours if mild cramps keep coming and going for more than a day, or belly pain comes with fever, painful peeing, or difficulty doing normal activities or sleeping. If the pain becomes severe or you feel much worse, get help sooner. Repeated pain also deserves a clinician’s attention, even when each episode settles.

If pregnancy could be possible and you have pelvic pain, seek medical advice now. Cramps and a missing period cannot confirm or rule out pregnancy. You can ask to discuss your concern privately with the clinician.

You deserve help with the pain even if bleeding never starts. For a way to describe a familiar cramping feeling, see what period cramps can feel like.
